Do you suffer from any of the following conditions?

- Liver problem or history of liver disease

- Hypothyroidism (an underactive thyroid)

 

- Kidney or urinary problems

- Hereditary muscle disorder in yourself or a blood relative

Are you taking any of the following?

- Itraconazole, ketoconazole, posaconazole or voriconazole (used to treat fungal infections)

 

- Erythromycin, clarithromycin or telithromycin (used to treat infections)

- HIV protease inhibitors such as indinavir, nelfinavir, ritonavir, and saquinavir (HIV protease inhibitors are used for HIV infections).

- Boceprevir or telaprevir (used to treat hepatitis C virus infection).

- Nefazodone (used to treat depression).

- Cobicistat - gemfibrozil (used to lower cholesterol).

- Ciclosporin (used in organ transplant patients).

- Danazol (a man-made hormone used to treat endometriosis, a condition in which the lining of the uterus grows outside the uterus).

Do you understand that:

- You should only order repeat supplies of medicines that have been prescribed by your doctor.

- You should ensure that you see your doctor for a review at least once yearly including a cholesterol test and liver function test.

- You should eat a diet low in saturated and trans fats, as well as high in fibre to help lower your cholesterol naturally.

- Combination with high consumption of alcohol regular could increase the risk of liver problem
